Madrid were starting to get desperate as the game neared injury time but they were given a lifeline as Vincius Jnr's relatively tame effort was spilled into Joselu's path by Neuer.
The substitute tucked it away to level the tie and then compounded Neuer's misery by putting the hosts ahead three minutes later.
Fans couldn't believe their eyes as one of the greatest keepers of all time made the most uncharacteristic of mistakes to cost his side their lead, after Thomas Tuchel had subbed off Harry Kane, Leroy Sane and Jamal Musiala.

Bayern never recovered from Joselu's quick-fire double and Madrid booked their place in yet another Champions league final.
Carlo Ancelotti's side showed once again why you can never count them out and Bayern will end the season trophyless for the first time in 12 years.
Los Blancos meanwhile travel to Wembley where they will face another Bundesliga opponent in Borussia Dortmund in the showpiece final.
